Accessible Information Standard
At Lambert Medical Centre, we want to make sure that all our patients, including those with a disability, sensory loss or impairment, can access and understand the information they need and communicate effectively with our practice.
The Accessible Information Standard ensures that patients and their carers receive information in a format that is accessible and appropriate for their needs. This may include large print, easy-read information, Braille or other formats, as well as additional communication support.
Tell Us How We Can Support You
If you have any communication or information support needs relating to a disability, impairment or sensory loss, please let us know. You can tell us your preferred communication method and how you would like us to provide information.
Please complete our Accessible Information Standard Questionnaire
Making Healthcare Information Accessible
We aim to make sure that everyone can access and understand the information provided by our practice and receive any communication support they may need.
This could include:
- Information in large print, Braille or easy-read format
- Information provided in a specific way or format to meet your needs
- Support from a British Sign Language (BSL) interpreter
- Support from an advocate where appropriate
Communicating With You
We want to communicate with you in a way that works best for you. Please let us know if:
- You need information in a particular format, such as large print, Braille or easy read
- You need information to be provided in a particular way
- You require someone to support you during an appointment, such as a sign language, interpreter or advocate
- You use another communication aid or tool
If you have any communication needs, please speak to a member of our team so that we can make appropriate arrangements and ensure you receive the support you need.
