If Death Occurs at Home

1. Telephone the practice on 01845 523157 to inform to GP.

2. The GP will liase with the Medical Examiners office and complete the death certificate.

3. The Medical Examiner will contact next of kin with updates.

We send the death certificate direct to the Medical Examiner, there is no longer a paper certificate that is collected. The Medical Examiner will confirm with you when you can register the death and arrange booking the date for the funeral.

 

If The Death Occurs in Hospital

1. Contact your preferred funeral director.

2. Collect the certificate from the hospital

The hospital generally sends the GP practice notification but if you are able to inform us it would help to ensure we have the correct information.

Notifying agencies/organisations

When someone has died, you may need to (or wish to) notify several different organisations about the death.
