COPD Clinics

If you suffer from COPD you will be invited to book an annual review phone call where the Nurse will:

  • Discuss any symptoms you are experiencing and their impact on you
  • Review your Inhaler technique
  • Give advice about your treatment/inhalers and make any appropriate changes
  • Advise on signs to monitor which may signal a chest infection & the appropriate action to take
  • Ensure the one-off Pneumonia vaccine has been offered
  • Ensure the annual Flu vaccination has been offered
  • Offer lung exercise classes (Pulmonary rehabilitation) as/if appropriate

Stroke Review

A stroke review is an appointment with a Practice Nurse once a year to review your condition. They will monitor your current health status and discuss options to keep you as healthy as possible. In a stroke review you will have your height and weight measured, have your Blood Pressure measured and have some blood tests. The Nurse will also discuss any medication you may be on.

Heart Disease Review

In a 20 minute Coronary Heart Disease review you will have your height and weight measured, have your Blood Pressure measured and the Nurse will take some blood. The Nurse will also discuss any medication you may be on, also diet and exercise.

Asthma clinics

Asthma patients will receive an annual review. This may be over the phone or on collecting medications from the Pharmacy. There is also a form via the website which can be submitted to include the details for your asthma check. Some patients may be invited to the surgery for an appointment

At your annual review the nurse will:

  • Discuss any symptoms you are having and their impact on you
  • Review your treatment and Inhaler technique – please bring your inhalers with you to the appointment
  • Perform a quick blow test called a ‘peak flow’
  • Provide advice & support with stopping smoking as required
  • Ensure the annual Flu vaccination has been offered
  • Discuss & consider the one-off Pneumonia vaccine if appropriate
  • Provide you with a personal self-management plan

Diabetes

If you have been diagnosed with Diabetes you will have an annual review once a year. Initially you will have a 30 minute appointment with a Health Care Assistant who will take blood, take your blood pressure, and do a foot check. Please bring a urine sample to this appointment. The results of this appointment may be sent you in a text if everything is ok an you don’t need another review until next year, or you may be asked to book a telephone appointment with a Diabetic Nurse.

Six months after your annual review you will be asked to come in for a 10 minute blood appointment to monitor your sugar levels. The Diabetic nurse will review these results and you can phone up after three working days for the results.
